Pilot and Controller Communications: Incidents Reported to the NASA Aviation Safety Reporting System

The NASA Aviation Safety Reporting System (ASRS) receives voluntarily submitted aviation incident reports from pilots, air traffic controllers, and other participants within the National Airspace System. The incident reports are evaluated and categorized into categories describing the incident event. One of the many areas that are evaluated in these reports is communication or information transfer issues. Information transfer issues represent one of the largest categories of problems contained in these reports. The present paper describes ASRS 1993 data on eleven communication problems areas. The communication problem areas of Readback/Hearback and Phraseology were further evaluated for (1) the types of reported incident anomalies associated with these problems, (2) the evaluation of who was the original reporter, (3) who attributed to the primary problem, and (4) the phase of flight where the incident occurred.
